While hypogonadism can cause tall height, and definitely will give truly narrow shoulders, that isn't the case of "regular tall guys" shoulders.
Talking guys like basketball players, guys who played all through highschool and college probably had normal levels of testosterone growing up, which is basically a requirement for them to be athletic what so ever.
What you guys are noticing is that taller guys tend to not be as wide, compared to their own height, as normal height guys.
Biacromial width is the term. Acromion are the ends of your shoulder blades, that attach to the outer portion of your collar bone. The total bone width at the shoulders is the biacromial measurement.
The normal growth pattern of height, the biacromial width also continue to grow. BUT it does NOT grow in a 1:1 ratio with height.
So, say a guy grows 10%, he only grows 5% wider.
This makes him look more narrow in pictures, but in reality his absolute width is still increasing.
There are some exceptions and tall guys who have biacromial width like a jumbo jet, but typically they get a more narrow look, even though, again, their true width has still increased.
This is part of why a lot of tall guys have a Y taper, instead of a V taper.
The guy in OP picture also has massive trapezius muscles, which can give the illusion of being more narrow too.
Shaq, for example, has the same kind of structure, except he didn't have the over sized trapezius.

I'm a twig and I do "hammer curls" with 35 pound dumbbells, not perfect form but swing them a little bit forward before pulling up. Can do usually do 2-3 reps of 15-20 of these. Also do two arm tricep extensions and shrugs
But after I do them, I have some soreness in my elbows and they even click sometimes. My body can't even hold up to do basic arm lifts. What the fuck do I do.

How does one begin to be a runner?
I'm 6'5, lifted for years, and I'm ~18%bf. I used to run while playing high school sports but I have patellar tendonitis and even a mile or two of running absolutely kills me and makes my knees hurt for days afterward. I know I would benefit from the cardio of running but my knees always hurt after even 1 mile and I always have to spit giant gross balls of phlegm. A girl that I've been seeing has been asking me to go on runs with her but I'm scared that I'll have to puss out almost immediately. How do I prepare for this /fit/?

I am the same height with similar patellar issues. Broke and dislocated it playing football. I still get several dislocations a year.
First things first, get your quads and hamstrings stronger. I walk everywhere around town so it's 4-6 miles walking daily. Knees are tender but healthier than a year ago. Baby steps man. Don't run honestly, it'll destroy your already weakened joint. I use the elliptical if I have to do excess cardio.
The grim reality is talk dudes have shitty joints. Just go on a casual jog with your lady but I'd explain health issues.
