I do preacher curls which look like this, shoulder width elbow placement, supinated grip, etc

Do them with a dumbell. One arm at a time, FULL extension. This should solve your forearm pain.
Use the machine instead if it's available at your gym. Free weights feels awkward for me as well

Some of it is training, but I do believe it's a lot to do with genetics. Honestly, I'm just starting lifting, but I'm 235 and fat. I have shoulders broader than my waist, which I think will look pretty decent once I lose some weight.
