Isaac, Rex, Suicide, and Alfonso are stood in a circle. It’s a once-in-a-lifetime thing, and they don’t even notice it, but a dialogical rush occurs whereby the conversation spins perfectly clockwise, starting with Isaac (then Rex, etc):

So I assume this part is meant to convey the way in which the characters (4) are seated as well as the manner in which a small aspect of conversation happens. It starts with naming the characters, and then pushes into "once in a lifetime", and "dialogical rush" which then continues to confuse the reader. A better way to do it, as far as my opinion goes, is to list the names like this:
Isaac,
Rex,
Suicide,
and Alfonso stand in circle.
The "are" is clumsy, so I removed it. Then, write that "the conversation started from the top, with Isaac, descending down to Alfonso, (insert something with which to convey the rarity of this top to bottom structure)."
The sidenote of Alfonso seems superfluous, or extra. Heh, top to bottom.

It feels as if you are writing, following a train of thought into the stratosphere, where the reader cannot breathe because you have removed any air of interpretation or spacing.
