Depends on the outcome of the ruling.
If I sense a ruling is pissing my players off I change/soften it. It's their game I just want the illusion of difficulty and danger to be convincing.
Had a great bit in curse of Strahd the other day when my group of 3 level 4's woke up 6 vampire spawn at once. After they started getting close to death an panicking I had the city noble they had been courting earlier turn up with his elite guard and even the odds.

I ran the playtest for a game store. It's the same 2d20 system that Modiphius uses for all its RPGs. It's very abstract and mostly based around passing around the three different kind of plot points. The way it's structured, there's a lot of metagaming built into the game itself. For example, you can take as many defensive reactions (dodging or parrying) as you want in a round, but each time you do you give the GM more doom points, which are used to make enemies win initiative and fuel their special abilities. So you may find yourself in a situation where you want your character to just stand there and not defend himself, for a reason that doesn't make any sense in-game.

Can't beat orcs and goblins for your Dark Lord's conquering needs. Zombies and Skellys are classic as well, but a Demon Army is usually the kind of thing the BBEG overestimates their ability to control.

The BBEG in my setting is a necromancer that's made it a cultural norm in his country for people to "Join the Army" after they die. You have your funeral, they extract the bones from the corpse, burn the rest, the family puts the ashes in an urn or whatever, and then its Raise Dead, off to conquer in the name of the homeland!

Humans with weird beliefs and erratic behavior. Cultists, pretty much.

My last campaign centered around fighting a human crime ring, led by a sentient blood mist in a glass tank that possessed people. Its major objective was to end all death- which wouldn't have ended too hot.

The party occasionally had to stop dealing with them to deal with the Loveless- a bunch of people convinced that affection is dangerous, since a broken heart is the only thing that can't be healed by magic.

Party wiped during a random halloween side-excursion.

This time around, we're a continent away, dealing with an incursion by the Crimson- that same crime network-turned-kingdom, which had since annexed the Loveless as the priesthood to their own new religion.

Basically, I got a free ticket to making these people weird and hard to predict- it gave me a lot of flexibility in setting tone session-for-session.
