I loved Kamigawa, but I'm a casual player. I thought it was great, but all the competitive players I knew hated it.
IIRC it was something to do with a lot of the blocks mechanics being very inward facing. All the real depth it had only really functioned in block constructed, and the standard with Kamigawa sets mixed with others just didn't synergise well at all.
It's a shame. I loved the setting and the theme and how characterful and flavourful so many of the cards were.

The sales of it were so poor wizards gave away a box to stores for every three they bought and they STILL didn't bite. After Mirrodin, the playerbase had some expectations for coolness and power level that kamigawa only meets in theme (with some obvious exceptions like sensei's divining top).
Working at a place that has sold magic for years, it seems to be a consistent problem with top-down designed sets. It brute-forces mechanics to fit a theme and they're usually underwhelming. Combine that with Masques-tier hard act to follow syndrome and you end up with a lovely-looking block that few really liked.
Why did Wizards get rid of commenting and rating cards on Gatherer? It's been like 4 years now.
They print shit cards and don't want people using their site to make new players aware of what they're missing.

I take it on a case by case basis based on the campaign and the character concept a player presents to me, but I tell my players not to EXPECT that they'll be allowed and to have a plan for a PHB race/class/whatever if their UA idea doesn't fit with the game.Mystic is always banned in all it's forms because fuck the psychic-pokemon "create an anime character" class.

Why do so many shit games confuse quantity for quality?
Like, you'd think that all the time spent not designing rules that fundamentally don't work by design (look at 3.PF) would've been spent actually designing a decent system where every part works in tandem with one another.
And before anyone jumps down my throat for shitting on 3.PF, rules-lite systems suffer from the same shit as well if the mechanics of the game fundamentally oppose one another because the devs don't know what they're fucking doing (look at Dungeon World) and even tabletop games as a whole can suffer from overly complex bullshit that takes a paragraph to explain what can be said in a sentence (look at Yu-Gi-Oh).
I guess what I'm asking is, why do so many devs just not get it?

I get the feeling that it's you who doesn't get game design. You have to be very clear with your rules, and you have to cater to anyone and everyone who might play your game. Which is a major pain in the ass.
You hate things that are popular and allow that to alter your entire world view. You're basically just a contrarian cynic, which is why it's hard to take anything you say as anything except exaggerated hyperbole.
I hope that answers many of the questions you have. I just wish you'd ask those questions, instead of the loaded ones you choose.

everything I know about radiation tells me if she's using it to fight she's either gonna cook them from the inside out or make their skin slough off after their chromosomes are destroyed. How can I fluff nonlethal damaging radiation?

Nonlethal means "not going to immediately kill you" which is why too much of it can still kill you.
Look up radiation sickness, apply it quickly.
That said, the player chose a fucking AWFUL weapon that will have massively long term consequences. You may want to convince them to just go full Cyclops instead, anon, if this is a heroic setting.

The PolyHero dice are actually kinda cool, although the initial production run wasn't the best quality. I'm looking forward to receiving my Wizard set, which sounds like it'll be better quality overall.

By playing with a group that understands the lore? Also, you might to consider what Green Ronin did with their Conan D&D books and make it so everyone, even monsters, get 2 HP max per level so everything is super deadly and serious

Encourage your party doing cool stuff and let stupid things they do slide instead of needlessly focusing on them.
I'll give you an example of what not to do - once, my character decided to bury an ally of his instead of just leaving his corpse out in the open. In order to do that, he had to carry it with him through the whole city, and literally every NPC took notice and started buggering him about that, when it would be much more dramatic to just let him go unmolested and let him bury the fucking body.
