Depends when and where within "medieval Europe". Theres still surviving 15th century local laws for places like London, York and Coventry that make it explicitly clear that people need to keep the streets clean and tidy and increasingly severe penalties for not doing so. One of the first things Edward IV does after seizing the throne of England is to fire the guys in charge of carrying the shit out of London as they had been taking the money but not actually doing anything.

Flash forward a few decades thiugh and Erasmus describes people pissing on the floor of their own house and the floor being caked in filth and matted coverings that had just been layered up, when he visited England.
