Hey guys it's me again, been a eunuch since I was 17 and I need some hormonal advice. Sorry, that picture isn't much better. I understand this probably isn't something you want to hear but your bone structure would need changes to be considered fem. Since you're 20 HRT might still help you a bit, but in all honestly you will need surgery if only just for the bone.

In my own personal case, I started HRT looking quite masculine at 22. I now get told I look cute and fem, however when people say this they almost always universally add "except for your chin". Hormones did wonders for me, but I know that I need to get surgery on my chin. I tell you this because you will probably find yourself in a similar position; if you do start HRT the lower portion of your face will likely improve and may look fem (I can't say for certain), but you will need surgery to correct your forehead and hairline.

Sorry to sound so negative, but it is honest commentary and in no way meant to discourage you from trying. I definitely recommend trying to be fem if it something close to your heart, never give up on your dreams; just make sure your expectations are realistic.

The raloxofene may or may not lead to some feminization, and the testosterone would lead to some degrees of masculinization, likely in rough proportion to any muscle gain (technically it is 1:1 for testosterone, but that is because the ratio is standardized to 1:1 for testosterone, and everything else is measured against it). Alternatively, rather than using bicalutamide and testosterone, one could use some androgenic-anabolic steroid that is already having more of an anabolic effect than androgenic. That would be the sort of question you would want to ask at /fit/ though. Remember, it is normal for a cisgender female to have some androgen receptor activation anyway, but do try to determine which steroid to use to try to get muscle without masculinization, if you are wanting to keep it to two drugs. If I recall correctly, it binds to the correct estrogen receptor in ass fat to do things there, but it was uncertain if it also binds to the needed coactivators and corepressors to have the correct gene regulation for a female-typical ass fat distribution pattern. The ultimate effect of these AAs comes from preventing test. receptor binding. Spiro/cypro decrease/block testosterone production, preventing there from being testosterone to bind to the receptors. Bica blocks the testosterone from binding to the receptors, also preventing testosterone binding. Just sitting in your bloodstream, testosterone does nothing. It's merely a chemical signal that tells your cells to start masculinizing once it binds to a receptor. You can mitigate masculinization by either decreasing the strength of the signal (spiro/cypro) or by preventing the signal from being received (bica).

It seems that bica can block masculiziation pretty well, and I would say perhaps better than cypro/spiro, because bica has no activity when it binds to the androgen receptor, whereas spiro and cypro actually have a slight propensity to activate the receptor rather than just blocking it (but this is much weaker than testosterone). The only exception to this is the testicles (not the dick, I repeat not the dick -- so please stop spreading the meme that bica spares the dick from demasculinization), which according to wikipedia contain such high localized concentrations of testosterone, that bica cannot hope to block it even at high doses. The consequence of this is that taken alone bica can spare sperm production (but not dick function, again, as bica can easily enter the dick's blood and block testosterone binding there). Some people say that bica doesn't affect libido because it doesn't enter the brain, but according to wikipedia it does enter the brain and blocks testosterone binding there, and people do see decreases in libido on bica alone, sometimes at least.

In any case, taking estrogen (or certain SERMS [selective estrogen receptor modulators] I would guess) alongside AAs will reduce testosterone production either way.
This is because estrogen suppressed the production of the hormone GnRH in the brain. GnRH indirectly causes the gonads to produce sex hormones (testicles to produce testosterone in bio-males). So even without AAs, after estrogen suppresses your GnRH production, your natural testosterone production will drop in turn. Of course, this depends on the amount of estrogen taken.

I would also like to point out that cypro/spiro have other mental and physiological affects other than simply blocking testosterone binding/production, whereas bica is more "targeted" specifically to blocking testosterone binding.

Overall I think one can expect good feminization with minimal side effects (besides boobs if you consider that a side effect) by simply taking estrogen and bicalutamide. I don't think there's any evidence/logic to suggest that spiro or cypro are more effective. And personally I think bica has the potential to be more effective.
