I am going to try a theme with the issues tomorrow where every issue will be a "done in one", meaning every issue will serve as a complete adventure from start to finish. I have some already set aside (mostly Adventures of Superman webseries), but if you can remember some specific Superman issue numbers that are in fact done in ones, POST THAT INFO and I'll do my best to track it down.

yes, it's in one of the 7 issues that form the main story of Forever Evil. After the Crime Syndicate takes over - I forget the specific specifics - but he should have been invited, like all the main continuity Earth villains - to join up with the Crime Syndicate, but they would have been below them (something Lex wouldn't have abided, most likely, especially since the titular head of the CS is Ultraman is just an evil analogue of Superman) - so Lex is trying to sneak back into his own headquarters for his secret weapon(s), which he does get (I won't even spoil that part) and he's helped in this by a security guard.
This was written after the Season 11 comics had been out for a while, so there's no way the writer of Forever Evil didn't know about it, and when he was asked about it once at a Con or interview, said something like, well THIS Otis is from the movies (well so is Season 11!) but yeah it's a pretty horrific death for all the people who are trying to say that the current Luthor (Superlex) is not a bad guy and didn't do anything wrong, etc. etc. and even Superdad couldn't find anything on him, so leave Lex alone, yadda yadda.
I don't think any of the post COIE to pre-Flashpoints comics, even one-offs, ever had an Otis.

If you are doing the issues as they appear in the trade, this is actually co-written with Busiak, as I had offered to find the actual issue numbers for you.
So I will try to find you some one or two issue Busiak stories from around this time frame that you might be able to use (he did a Jimmy Olsen issue, but it doesn't stand out in my memory as being strong).
