The enitre premise that there are 4 outcomes stems from the idea that T H and H T are seperate results. Therefore, if we're taking out T T we also have to take out either T H or H T depending which of the coins landed heads, because if say the left one lands first as heads, the outcome couldn't POSSIBLY be T H.
