I assume there are prizes for winning tournaments that involve making your own card. The best duelist have fully unique decks do to each card being a tournament exclusive. They're probably also tied to a Duel ID to discourage the secondary market. Every quarter year there is a vote as to which retired player cards are turned into "staples".

In Arc-V, at least, they just sort of...pull them out of nowhere. Yuzu's Melodious Fusion monsters didn't exist until after she practiced with Sora, where upon they just sort of came into being. Same thing with Gongenzaka's Synchro monsters.

The Odd-Eyes/Dimension Dragon evolutions in general can sort of be explained by 'zarc magic, ain't gotta explain shit'. Once the Lancers became a thing, Reiji probably made cards for them, too. But there's still a lot of unexplained shit. I guess if you're a good enough duelist and know the method you can just will your cards into existence.

The last time Reiji tried to make powerful disbalanced cards, it came to bite him and nearly costed him the duel.

In DM, there were shops that would sell them. Later on in Arc-V, they are almost magical, since you can't simply print a piece of paper and use it as a card. Kind of each character can make the cards exist out of need, sometimes they even show the duel disk shitting out a card that the character never knew it existed, because the card just materialized right there.

Also, in the anime, the cards doesn't come with printed effects, so understanding what a card does is a lot more difficult, and the characters seems to "feel" what a card does. This is shown by Yuya in Arc-V not actually knowing how to pendulum summon despite doing it out of need.

Yes. Because the second chain doesn't start until the first one have resolved. After the resolution of the first chain, the monster will have already changed controller, but it will have still been just recently summoned so its effect still goes off.
