It's hard to say, because the guy that wrote seems to have made the hardest skull combos go hand in hand with the shortest levels. For example, the Iron Skull is a bitch, but it's only registered here to Floodgate, which is probably the shortest mission in Halo 3.
Tayari Plaza has a really tough 3-skull combination, but that's the smallest level in the whole series.

I'm going to go out on a limb and say the hardest in theory here is Exodus with Black Eye and Famine.
