There aren't any whale bones or anything other than maybe the oil canisters to suggest it's a whaling village, and its inspiration, Innsmouth, was an impoverished fishing town.

I like jsf's take on it, that it was an poor fishing village that was revitalized by the parasites they harvested from Kos.

Think about it, it's a hamlet, a village without a church, they had no religion so whatever culture they had was probably scoffed at by whoever they tried to trade with. Then Kos washed up, now they venerate the source of their communities bounty and ascension, and their wives they harvested from her worship her as their ancestral host.

I'm sure there's treatments beyond popping pills, though. I'm on pills for life. Immunodepressents might help stave off the worst of the effects, that's what I'm on. Basically lowers your immune system to a specific state where you don't die from colds but your immune system doesn't shit the bed and rot your intestines/destroy your bones. I honestly haven't noticed a single bad side effect of them and I'm on them for seven years and running.

This. I like how it features classic horror monsters like vampires, werewolves and witches without straight up callin them that (with the exception of Hemwick). I don't think a single npc or item description ever calls the beasts "werewolves" or "wolfs," which is neat because it doesn't limit what kind of beasts people can become (like how ludwig was half man half horse).

Isn't the blood of Great Ones the Paleblood you're seeking in the first place? I doubt that's what you were administered at the start; the man simply says you require *Yharnam* blood as part of the contract. e.g. outsiders can't be hunters without Yharnam blood in them.

The search for Paleblood allows you to "transcend the hunt" by becoming a Great One yourself in the Moon Presence ending.

What I've been thinking about recently is the Blood Minister and the contract he mentions. Is the contract a hunter's contract, and if so is he in collusion with the hunters? Is he selecting patients to trial the transfusions that test their immunity to the scourge?

There is no dream world, the Waking World is the same place you go through when you are in Yharnam.
Then there is the Hunter's Dream and all the Nightmares, which are alternative dimensions/realities where one can either enter with their real body (like the Hunter does) or by sending their conscience (like Micolash, the Old Hunters and everyone else who is dead in the Waking World does). They are inspired by Lovecraft's Dreamlands, which worked exactly this way.
In the Yharnam Sunrise ending you are just freed from your contract and thus lose the ability to go back to the Hunter's Dream every time you die, meaning if you die again you die for real, just like all the other hunters who stopped dreaming (like Djura and Eileen).

But I initially took the nightmare it was referring to as Yharnam under the influence of the moon

Even what appeasr to be the sun that's in the sky up until Amelia looks unearthly, the edge of it is lined with a shadow, like some kind of eclipse. It's the antithesis of the light that shines off the edge of the moon in the Hunter's Dream.
