The main thing with the confusion gate in the original for me is that the music less grating, and there's no Eden that gets revealed to be the area, it's like that as soon as you enter it..

The puzzles for the most part are the same, but there's some notable changes (chamber of life comes to mind). Also the original has more obstacles that you can destroy only with specific weapons, while the remake made basically most of them destroyable with any given weapon (I think?).

Some enemies are different too, such as any Anubis enemy in the room creating screen-wide field that damages you just by being in the room, as opposed to the beam they do in the original. Bosses are also subtly changed, though I think they're a bit easier in the original.

Farming money is easier at the start of the game than in the remake, because once-per-load of a save any pot that drops coins or sub weapon ammo refills.

The main major difference is how the MSX works, because it's quite literally a MSX in the original where you swap one or two ROMs to achieve the effects you get in the remake, and there's a shit ton of them.

I'd argue a lot of it comes down to aesthetic preferences and some quality of life changes in the remake.

It did it for me
I watched my sister play the whole game. Everything except the late game is some goofy dialogue, and kind of comfy environment. The bad guys were acting like Team Rocket from the Pokemon cartoon.
But then the last level went full Silent hill with the bad chick acting evil as shit, the level was creepy with a red sky and some creepy chorus as the level theme, bloody machinery and so on.
We never managed to got passed that whole final boss

I played pic related as a kid back in '09 (i was 12) after my cousin with internet got me some games in a CD.

I couldn't understand jackshit of Japanese but it at the time i got sorta freaked out by some areas but just continued anyways.
It took ages to complete the game but when i did, just, holy shit.

Yeah, I never understood the panic about "return the slab". Wasn't unnerving for me and Eustace's dialogue made it pretty hilarious to be honest. What really was always giving me creeps in Courage was this alien guy in trenchcoat which sucked some shit out of Muriel through a straw.
