My personal theory is that the Yharnam we play through in game is a dream. Not like "it was a dream all along" but just another real physical dream like the Nightmare Frontier.
I believe that the real Yharnam does have a hunt and does have a beast problem, but on the night of the full moon this dream Yharnam is created. Whether people in the real world are aware of it or not I don't know.
The people in the dream are real though. Whether they are alive in the dream and in the real world, or physically pulled into the dream like ourselves I'm not sure. They know the hunt is on and they know strange things happen during the hunt, which is why they try to stay indoors.
This dream Yharnam lasts until someone ends the hunt. The hunt needs to be ended because the night goes on and the hunt goes on and regular people are suffering because of it.
I think the Moon Presence creates the Yharnam dream on the night of the full moon. It seems to like hunters so it might create the dream so that proper rune-inscribed hunters can be made, or so hunters have a good place to hunt. When the dream ends it gets a new hunter, or at least it would if Gehrman would stop killing them.

The representation of the two things in the nightmare could be the hunter's third eye/subconscious trying to tell them that the Doll 1) is not as she seems or may even 2) be something they must avoid altogether. Fear of the unknown plays a big part in pretty much everything the lore stands for.
1) Throughout the game the more grotesque beings/truths aren't completely/correctly perceived by the player until later (Mergo's crying, the Amygdalas, various appearances, move sets and behaviors of mobs and bosses etc) or can never be fully perceived in the playthrough (Mergo's Wet Nurse's full visibility). The Doll's entire being and capabilities might be more than the hunter could ever imagine.
2) It's possible the hunter is subconsciously afraid of losing their humanity from gaining inhuman knowledge and letting the Doll turn the ancient blood we accumulate into our strength and being. Lovecraft's work often stresses the fear of losing one's humanity, as if the process were a slippery slope; Piece by piece we're hustled out of our humanity in pursuit for inhuman knowledge while being pushed towards the edge of transforming into a being that is enigmatic and inhuman where they'll be ataraxic and lucid. Maybe devoid of physical form.
For the hunter, it's their wild curiosity and uncanny desire to unravel the truth that inadvertently draws them towards that end (kill MP and become and infant Great One). In another scenarios it's their normality that allows them to surrender their life in the dream to forget everything that happened after taking Yharnam blood so they will awaken at dawn return free from the dream with their humanity intact, and in a position where they will hopefully "find [their] worth in the waking world". Lastly, the hunter may take Gehrman's place as the Moon Presence's slave by killing Gherman.

In DS1 Lost Izalith was crap. After Fotfg and Bastille in DaS2 everything was mostly crap until the DLCs. In DS3 after High Wall everything is tedious crap until Irithyll with the exception of maybe Cathedral.

Bloodborne doesn't feel like it has any irredeemably bad areas that would feel like they dragged on too long or were hideously annoying.

Yahar'gul maybe comes the closest but you can skippity dippity doo through it really fast if you know what you're doing.

If the areas with Winter Lanterns were mandatory in Mergo's Loft, my answer would probably be that.

Forbidden Woods is imo cream of the crop of Souls nature level design. They managed to make the area feel teally natural while still making it play like a real souls style level with physical shortcuts and making it loop into itself nicely. A lot of hidden stuff to explore.
