I think they USED to be both sections of the Church, and probably friendly. But Mensis seemed to have gone rouge quite a while before the game is set.

I have a feeling there is a huge chunk of lore missing from the game involving the worship of Amygdala. Through external sources we know that they are "false gods", and the game even included that term as a message. Mensis has a statue(s?) of one, and they are everywhere in Yharhagul. Maybe Mensis scholars learned from the Amygdala how to enter the nightmare, thier cage-hats even resemble Amy's head.

If the church/choir deemed them "false", preferring to instead study Eribritas and (inadvertently) worship Oedon, it would explain the split between Church and Mensis. Who seemed to vanish into the nightmare a long time ago (possibly Old Yharnam burning night?) leaving a bunch of Pthumurians and kidnapping hunters to look after the Unseen Villiage. But whilst in the Nightmare thier ritual wasn't working because Rom acted as an interdimensional cock-block. After she is dead the Ritual is revealed and Bloodmoon happens and everything in Yharhagul starts resurrcting (Yharnamite enemies represent all the townsfolk killed) and Pthumuruians Desperetly try to rebirth Mergo into a new body (The Ome Reborn).

This obviously doesn't work, and you reach the Nightmare of Mensis, find Micalosh who's seemingly the only Mensis scholar still alive and trying to ascend. Then you slay Mergo and stop the ritual that was pissing off the Moon Presance,

The Hunter's Dream, Nightmare Frontier, Nightmare of Mensis, all those areas are areas in a larger area called the Nightmare, an alien dimension where the Great Ones come from. It's a layered space, as best we can understand it, with the Hunter's Dream being a bottom-most layer. It's an area controlled, not created, by the nameless Moon Presence. The Nightmare is just a place with different named areas, they all continue to exist regardless of Great Ones.

Micolash was basically the manager of the Nightmare of Mensis, their ritual HQ. Mergo was the infant they were beckoning the Moon Presence with, you kill it, you end the ritual.

Rom is a mindless tool used by Mensis to hide their ritual.

Amygdalae are just aliens, we don't know exactly what they are. One theory is they're ascended Pthumerians.

Beasthood is caused by vermin, small writhing creatures found inside the various strains of blood the Church uses and doles out amongst the people. People use blood, get vermin in them, become beasts. The blood is ultimately alien in origin.

Not all of this is strictly true. The different dream worlds are not all necessarily connected and were created at different times. Namely the Hunters Dream, which was created by the MP in a contract with Gehrman/Lawrence. The Great Ones also don't necessarily come from these worlds, but can create and inhabit them, but they don't strictly exist in one specific place. Oedon for example doesn't have a physical form and is almost more like law or phenomena surrounding blood.

Beasthood is not necessarily caused by vermin. It's caused by a combination of use of old blood, stagnation and corruption of the blood, of which vermin are another sign, and indulgence on the subjects part(which is also resonant in the use of blood).

Queen Yharnam had blood so corrupted by eldritch forces that Oeden manifested in it and they conceived Mergo. It seems like Mensis somehow took Mergo for use in beckoning other Great Ones like the Brain of Mensis/Maybe Oedon.

Rom was an attempt to create something like the player becomes in the third ending but they didn't have all the ingredients so it didn't really turn out well.

Amygdalae aren't really elaborated on at all in the game. Common sense says they're servants/aspects of Amygdala.

The MP is just a Great One, the whole idea is to ascend humanity by contacting the Great Ones. Byrgenwerth started it, the Church continued it, Mensis, a section of the church, decided to take it further and continue Byrgenwerth's exploits.

Mergo is an infant Great One. All GOs lose their child and yearn for a surrogate, Mergo is to be the surrogate child of the MP. Mensis is using Mergo like bait to lure the MP and hopefully it'll ascend humanity.

Gehrman was the first hunter, he started the whole hunter tradition, what they do is clean up messes and keep the whole beast problem as under wraps as they can. He's ex-Byrgenwerth and lives unnaturally long since he exists now only in the Hunter's Dream. His real importance comes from the DLC and the whole Fishing Hamlet massacre dooming hunters to an ironic hell

Your character isn't given a background within the game. Pre-release info stated you were there looking for the cure for some illness. You can kind of just fill in whatever you want.

If you let Gehrman kill you, you get to leave with your life. It's the only real happy ending and one rarely afforded to Lovecraftian protagonists.

Gehrman is basically trapped in a hell of his own creation and is probably happier if you put him out of his misery, but he doesn't bear the player any ill will. He made a contract with MP to make the hunters dream and is living with the consequences, kind of like how Micolash can't leave the nightmare, there's nothing to go back to.

They're not 'dream world', the Nightmare is one large layered area. You can see Mensis and the Fishing Hamlet from the Frontier. You can see the Hunter's Nightmare from the Fishing Hamlet. It's alien layered space, but all connected. It's one vast area. They're manipulated and controlled by Great Ones, but not all areas are. Formless Oedon, also, is described as existing as a sound.

Gaint vermin exist in the Loran and Nightmare silverbeasts, as they do in the Headless Bloodletting Beasts. Vermin is a blood thing. Old blood causes beasthood, and vermin are to be found in blood. It's not a huge leap to make. This whole corrupted blood thing is because of different strains, like what Cainhurst have, and because of vermin.

No idea where you're getting the Rom being a proto-slug baby thing from, to be honest.

We don't get a good idea of MP's goals from the game, that's kind of a staple of Great Ones.

What we know is that he was beckoned by Gehrman and Lawrence with a 1/3rd chord and they created the Hunters Dream. What he gets out of this is unclear. It's possible that infusion of his blood is what creates hunters, and that as they grow more powerful he somehow gains.

We know he has an interest in keeping the Hunters Dream working as normal, as if you kill its caretaker without being able to take on MP, he makes you the new Gehrman.

That's all the direct information on him, the rest is speculation.

Dream world doesn't mean "not real" it's a general term for the alternate dimensions created by Great Ones. This concept is lifted straight out of Lovecraft. Were are you getting that they're ALL connected? There is no link between the Hunters Dream and the Nightmare Frontier, and we also know the origin of the Hunters Dream and that places in the nightmare can be shaped and created by Great Ones, and that it exists outside of "normal" space just like the Great Ones do. Oedon lacks physical form because he is an extradimensional being. He isn't literal soundwaves bouncing around the physical universe.

The Vermin come from Shinto principles. They represent the impurity within man, but that alone is not enough to turn them into beasts. Men don't turn into beasts without indulgence in old blood. The Vermin don't create beasts the same way mosquitos don't create swamps.

How are you confused about Rom?

The Church (and everyone else) seeks to ascend humanity to the level of Great Ones.

The Church found Kos and used her to create Rom (who Micolash tells WAS NOT ALWAYS A SPIDER and was "Granted eyes" and so "ascended")

So Rom WAS an early experiment in human ascension.

Rom also WAS a failure in that she is basically braindead.

I'm getting that they're connected because you can actually see the general layout of various areas. Lovecraft's Dreamlands was a fantasy world connected by regular travel on horses and boats and birds, mind you, as well as existing underground for the Ghouls and in dreams for humans. 'Awaken Above Ground' is the message on every single Hunter's Dream tombstone, implying you are somewhere beneath something. Also, Oedon is literally described as a sound in the rune description of Formless Oedon: The Great One Oedon, lacking form, exists only in voice, and is symbolized by this rune. Runes, as we know, are symbols of sounds.

It's the overuse of blood that makes beasts, yes, pumping it into yourself, day in, day out, filled to the brim with vermin.

Your assumption that Rom was supposed to be the same thing as the player character, where do you get that from, exactly? I mean, all you're saying there is right, but that's only supposition, we don't know what Rom exactly was supposed be, but we know what she's used for.

There's a whole bunch of knives in the Cathedral Ward, at the top of the alleyway where you talk to Arianna and Skeptical Man, search around there. I think you should be able to buy them, too after getting the Cosmic Eye Watcher badge in Upper Cathedral Ward. Also, you can farm them in Layer 2 of Central Pthumeru Chalice, from the poison knife throwing undead.

The simple answer is the Paleblood Sky that accompanies the Blood Moon after Rom, you had to seek it to find/reveal the Mensis ritual and complete your duty as a Hunter. This sounds dumb, but it's 100% intended to be the primary thing in the game where you say "ah yes, this was my objective all along". Miazaki talks about this in the interview in the guide. After this you accept Gehrman's offer and are freed from the Hunt (transcend) on a personal level.

However it also refers to the (nameless) Moon Presance, Miazaki also talks about this. Which all but confirms the Blood Moon and MP are connected. Here seeking paleblood takes on a new meaning, you wernt meant to ever see the MP, killing Gehrman was not part of the plan, if you can defeat it by eating 3 cords then you truly transcend the hunt, by killing what was causing it, you didn't just personally escape the Hunt you stopped it all together.

Any other interpretations Miszaki doesn't want to reveal or discuss to keep it open for fans and himself.

If my added bits about 2 different ways of transcending the hunt were intentional, then I think it's a nice double meaning. The only problem was no one knew they were meant to see the Blood Moon and instantly think of "a body drained of blood" as assume this world state was a callback to the beginning of the game.
