Most people don't get automatically put into war with Iga because they're either at war with Tenshi or they're at war with somewhere like Asai-Asakura. What happened?

To the best of my awareness, that's only a half truth. Iga declares war on you in ANY game (First isn't special for them, unless I'm mistaken) if two things are true:
1) You're not at war with anyone.
2) The Tenshi Sect monks have been massacred.
Usually, #2 negates #1 because the massacre causes the Tenshi Sect war, but it's not always the case. I've been meaning to dig deeper into this, but I think Iga's war is an end of turn event, while Tenshi is a start of turn. In my most recent playthrough, on one turn I had just beat Ashikaga, and by the next both Iga and Tenshi were at war with me.
