It's good against soldiers and demos. The extra damage you take ends up being redundant, as it's two rockets, pills, stickies, whatever that kills you.

And the pistol isn't really that good except for doing chip damage from afar, so the 5 health you get back is good when you're flanking and don't have the medic around
