This has been happening since DS1. It wasn't as prevalent in DS1 because it would only really happen with the iron golem boss, but in DS2 one of the first few enemies you encounter will do it (the ogre). And then the pursuer does it soon after too.

People make excuses by saying you need to depend on i-frames or the attack hitbox just barely touched you so it makes it okay for you to warp into a grab, it's like any criticism of the game gets deflected because everything has to be a challenge to overcome, which is good in some ways, but not at the expense of game logic.

Souls games are pretty fun. They're not bad because they have some flaws, every game has flaws. But this is a flaw. The animation and hitboxes for grab attacks are horrible and they have been throughout the whole series. Regular attacks are mostly fine.
