It does get confusing.
Plot wise, you are Zach and talk to an alternate personality, York.
Because of the trauma, the roles were reversed. Zach, the traumatized kid, retreated to the subconscious while York, unaffected, came out. That way, he'd go about his life without feeling bad for his past.
The player sees York. We see what Zach/York want's us to see. A confident, funny, enthusiastic young man. Only when everything is revealed, York goes off with Emily while Zach is left alone in the real world.
I think it's a red herring with making the player believe that York is just talking to the player, reffering us by Zach.
