Basically, he stops the cold war from going hot after it was exacerbated to the point of insane criticality by Dr. Atom pushing the soviets back far more than the US ever actually managed to do, what with being a near literal god. He does it by blowing up New York City with a fake elder god he grew in a giant vat, then filled with music, images, etc. from every abstract-horror artist he could find so that when it released it's massive psychic death shock, telepaths the world over would be struck with nightmares of some horrible other world. Soviets and NATO put aside their differences to work together to protect the world from interdimensional horror terrors. The entire plot of the book is how the 'heroes' are trying to stop him, even after it was made horribly clear that stopping him would have ended in nuclear war. Also they don't get the villain speech until 35 minutes after he launches it, and it's just the best villain line ever, what we've been waiting for every villain to say in everything we've read/watched up to that point. The simple act that he goes back to heal Alibert should tell everyone that he' has an unsound mind, since according to his doctrine it doesn't matter, and actively opposes his goal.

{spoiler] Rando wasn't the bbeg in LISA, he was only trying to protect the most important thing to the human race at that point. If you really look deep into it, you'll find that Brad was a horrible father. He limited Buddie's interactions with her "uncles" and then killed them later just for trying to prepare buddy for life in Olathe in a peaceful way. If you've played through the Joyful, then you'd know that Brad had kidnapped and subdued men, and then forced Buddy to kill them just because he didn't want a weak daughter. Also, Buddy wasn't kidnapped in the beginning of the game she was "set free" in a way by two men dressed in yellow. These two men dressed in yellow were part of Rando's army and were sent by him to offer buddy the chance to truly live her life. Rando and Buddie's uncles were the only people who really cared for her.

Brad on the other hand killed his childhood friends because they gave Buddy information, killed his father because he gave Buddy shelter. Brad also carved through western Olathe in a red wake of corpses, murdering almost everyone he met. He then abandons his followers and slaughters more people, people who were part of Rando's army, ultimately beating his adopted son (who he abandoned when buzzo cut his face off) nearly to death.
