Actually just VANOS service and the cooling system. Intake and exhaust cam bolts.. new seals, solenoid, oil pump disk. My old hub was fine. Rod bearings were done in the 30k range under recall.
I did the fuel pump too.
I bleed brakes yearly. Flush coolant yearly. Change oil every 5 to 6K mi. Check valve clearances every 15k (when I tracked her I had the valve cover off 3 to 4 times a year). Wash every 2 to 3 days. Remove wheels weekly for cleaning. Etc etc etc.
That's it.
Worth the upkeep.

You do the cooling system every 80 to 100k. Most do not fail during this timeframe. OE parts for belts and all runs $700 or so. Mine was $1300 but I bought a Mishi rad and OEM everything else. The same with the VANOS. A good 100k overhaul item. The least you can do is the solenoid which is extremely simple and only $150 after your core return. A full rebuild of the VANOS is under $1k diy.
Valves do need checked every 30k miles but it's easy as well. I've done it so much I can strip down to the valves in under 30 min. Adjusting is simple as well and not expensive either unless you get OEM shims.
I'm eyeing a mazda 6 V6 2011. I'm moving up from muh mazda3 (05). Should I expect about the same for reliability? The 3 had some suspension issues last year and usual wear tear items. Sadly automatic. The MT on the 3 was pleasing and i'm sad to lose it.

My 2000 540iT has 280K. Runs fine. Did timing chain guides at 200K, because I'm sure they were due (they were mostly fine) and VKG + trans service every 100K.
Car runs fine. Compression and leak down numbers are good and I'm not leaking anything.
Most people don't put this much maintenance into an E39, but my parents bought it new in 2000, and I bought it from them in 2004.
