The word Tsar means emperor (coming from Caesar). They are just called kings, because stupid anglos can't understand the distinction between empire (a powerful state, like british empire or mongol empire) and a country ruled by emperor, like a german Kaiserreich or Цapcтвo.

For a bit, they tried to get along with the Byzantines.
The Emperor title in Latin era was Augustus, the second most important position was Caesar.
Byzantines later recovered the latter and created the nominal title Caesar as the second most important within the empire.
To seal the peace deal with Bulgarians, the Bulgarian King was nominated Caesar, hence second to the Emperor only.
Then the deal collapsed but the Bulgarian King kept calling himself ''Caesar'' and claiming he was the rightful succesor to Constantinople for a while.
Must to Byzantine's hilarity because they knew Bulgarians stood no chance against the walls of Constantinople.
