You don't have to "activate" macOS at all. As long as your hardware is similar enough to Apple hardware that macOS contains drivers for it, and you patch your firmware to something it recognizes -- a practice that's really very simple, if you build with the foreknowledge that you're doing a Hackintosh -- you can install macOS without any further hassle. You even get updates.

Speaking of updates, macOS has never been, and never will be, so pushy about OS updates that it won't allow you to finish what you're doing. Windows, on the other hand...

Hackintosh preparations are not the same as Daz Loader. Hackintosh prep just tweaks the way your hardware works so that it's in keeping with what macOS expects, allowing it to function as intended. We don't have enough information to say that; it could have been given to another edge case, or Steve Jobs could've survived. There are lots of plausible hypotheticals we could throw around.
Alos, his diagnosis wasn't clearly given as terminal; he had a relatively safe form of liver cancer, and I kind of doubt any doctor would have given him a liver if they knew he would die in six months (it was actually two years).
