Probably something wrong with either the chemistry of the battery cells or the design of the battery controller or their charging firmware. Charging a battery isn't as simple as throwing some electricity at it, you have to vary the power input in response to heat and stuff like that. It has to be done in software.

The batteries must be bad. There's no way the phone can generate the heat necessary to make anything combust. Chemicals are probably mixing in the batteries and Samsung can't fix it in a timely manner so they're just killing the phone.

Battery bursting might not be from combustion, it might just be gas buildup in the battery. This can be caused by high temperature-- either from the environment, or from improper charging, or from defective battery cells, or some combination. That temperature could be way less than combustion temperature.

I have a theory
So right when these things started exploding Samsung pushed out a broken update for the Oculus Samsung Gear VR which caused all of their phones to suffer from extreme battery drain and to get extremely hot.
The Note 7 probably couldn't withstand this kind of abuse and fucked the battery to the point of extreme failure.
The reason that app caused this in the first place was it would get stuck in an install loop and there was no simple way to kill the process.

Samsung forced too much current onto their batteries just for the sake and quick charging them and that broke the batteries, they made a mistake in the Kernel and pushed it too far,.
The rerelease and subsequent destruction of the note 7 line proves that this was a design flaw not a battery issue.
Note 7 was re-issued with batteries supplied by the Chinese company Amperex Technology Limited (who also serves as the main battery supplier for the iPhone line)
