I've had no difficulties. Just make sure you pass all of the SMTP diagnostic tests on MXToolbox. As long as your VPS provider handles complaints about shitty users promptly it's unlikely their customers will get blanket blacklisted. OTOH if you run an open relay or something, you can probably expect to get shitcanned by your VPS provider.

Obviously, this only applies to hosting your own email server on a VPS with a reputable provider. If you use a cheap, shitty VPS which never takes actions against abusive customers - or you try to setup a mail sever on a domestic internet connection - your mails will go nowhere.

The number one thing to do is to make sure you are not relaying any mail that you shouldn't be. This is not hard to get right:
If the mail is destined for a user on your domain, accept it.
Otherwise, if the SMTP transaction is unauthorised, drop it.
If the SMTP transaction is authorised (i.e. your email client sent a correct username and password), relay it.

Finally, make sure your email server is speaking STARTTLS or SMTPS. Do not advertise AUTH capability otherwise. Your credentials will be sniffed.
