Recomendations
The interactions between body type and supra-thyroid need to be more explicit. Also the normie tier weight management logic triggers my inner /fit/izen, but I understand this is CYOA logic so all is forgiven. Same deal with how insomniac and sleepy interact. Do they cancel each other out or do you have to sleep 75% as much as normal?

There's kind of an issue with how the benefits are all divisible by twenty. There are a lot of different options that give you five or ten points, but no benefits you can take for that much. Consider giving some of the existing benefits and punishments different tiers, like having different ranks of that cost variable amounts, as well as adding more shit in general.

Also, it's super easy to just squeak by with just getting a few physical changes and just be a better guy.

A moderate problem is that, since you can only spend or gain 5 point increments during the first stage, you can easily end up with 5 points you can't spend.

Also, the final 3 options all seem pretty lackluster for what you have to put up with and the risks you take. Though maybe I simply lack the fetishes they cater to.

You're also missing the word "annoy" from the Touchy description and the final "Your" in Dolled Up should be a "You're."

I think the best thing you could do for it, apart from adding more options, is add some pictures desu. Pure text on a dull grey background isn't particularly eye catching.

The frame is pretty much the same as that other CYOA with a few tweaks. While I don't mind the extra points, it's a bit odd that the conventionally attractive "Feminine" face should give points, while all the comparable features in other categories (hourglass, bust size) make you pay. Seems a bit...torn in different directions at the design level from the original, but unwilling to change it all the way.

I still think it's weird that you have to pay so much for things liked red hair or green eyes (combined cost is basically half immortality, which is kinda ridiculous). But paying for "distinctive" traits like that was a core design theme of the original CYOA, so can roll with it.

The benefits/punishments section is basically the same as the original, only with a few more mandatory picks. Increased Lifespan, Wolverinemode, and STD Immunity are so compelling that a lot of the more interesting options get tossed aside--not to mention that the point crunching basically makes stuff like "brown eyes" or "brown hair" drawbacks that you take to achieve godmode. This was one aspect I felt the original balanced a bit awkwardly, but I'd understand not wanting to completely reinvent the CYOA. Also, it's always felt weird that life-derailing options are side-by-side with generic sluttiness drawbacks--and even give fewer points.

The Sell Out section is very problematic. RNG is always risky in a CYOA and complete stop-all-fun bonerkilling RNG has to be very carefully employed. This CYOA is so point-tight that we're strongly encouraged to alter the fluff of our personal appearance to horde points. One of the Sell Out options returns 4x more points than the most life ruining drawback and has a 50/50 chance of completely ruining your character. Fate worse than death levels. The lesser options are life-ruining insanity and character death. Why is this here?

I'm guessing you're looking for more +/- point options that give character and help build a scenario?

Maybe add some -10 benefits, because i ened up with 10 points and nowhere to spend them though i guess there would some rebalancing anyway and something would end up more expansive and those extra points would end up handy. Supra-thyroid is basically free with Endo as it negates Endo's main drawback so if you are not going for Ecto there is no reason not to take Endo over Meso.
