Trans people feel their sex (penis/vagina/boobs) does not align with their gender (male/female) so they feel they must transition to (male/female) in order to feel like they are finally in the right body. People often confuse sexuality with gender or sex with gender but I assure you these are all different things. A trans woman can be 'gay' before transitioning and be considered 'straight' after transition. It's not a mental illness at all.

I think the best thing you could do is make sure that s/he can at least get a legitimate diagnosis for gender dysphiria. That means s/he has to see a psychiatrist that evaluates her mental state, reasoning for why she wants this and makes sure s/he understands what this could mean for her life and health. There are some who will breeze through it which is why it's important for her to speak to one who is thorough. This is not something that should be taken lightly and is best if she can have professional help throughout to make sure it's done correctly, safely and properly. As someone who is the other aspect of being trans, MTF, In absolute honesty, Anon, yes, this can potentially be more dangerous than it has to be.
I have been lucky in that, since beginning my transition, I've had nothing but support.
