Now, to middle man something you need to know the identity of the computer, people that have been caught this way is because they have been dumb fucks and gave away hints of their irl persona or exploits were used to reveal their real IP.

There was a case of some kid that got caught this way after the FBI generated enough info to speculate about who he actually was. They parked a few meters outside his house and middle manned his connection. to confirm he was the target. He was, got arrested.
