Treat it like Americans speaking English when they are playing Romans. It's assumed that they are speaking Latin, but in the show they speak English so that they may be understood.
If it's a fictional country, you'll never know what her native language sounds like if the creators didn't bother making a separate language. Jap or your native tongue would be acceptable I think.

It's kinda true, at least for Japanese couples. The younger generation uses it more though. Love is preferred to be shown through actions instead. Saying 好き or 大切 is a lot more common.
To Yunocchi I only say 愛してる in more serious, or emotional situations, and it still makes her go a little red.
