I've made a ton of creepy pics in photoshop over the past 15~ years. It's pretty effortless if you know where to look. This effect literally took two steps.

It helps to start with a pic that's already a little weird/creepy. To get something similar to the effect you used, I just applied the Mosaic filter under Filter > Pixelate > Mosaic. After that, I messed with the Threshold under Image > Adjustments > Threshold.
