If you're having trouble finding a concept try variations on a theme. What is scary about _____? It may not be something inherently frightening but it could have aspects that cause fear. Cars, for example. Past trauma (also a good starting point) aside no one is afraid of cars as a concept. But they are dangerous. They are very fast. They make loud noises from their horns and engines. Some motor vehicles have particularly grim tasks such as hearses and livestock transports. Police cars are purposely uniform. If you are hurt badly it is likely you will be taken to the hospital by an ambulance. You pick one or more mundane aspects and attach them to or explain them with a paranormal underbelly. I'm currently working on a short story collection of speculative fiction, could totally keep talking shop.

Just try to write about reality. Here is a meta introduction, though a small one: Humans accessed eternal life technology...thousands of years earlier than now. They mingled, cooperated, conflicted, and married humans not accessing eternal life technology.
