I should probably add here, I don't remember what I do whilst blacked out, it comes back as if its a slideshow or a dream, I only recall what happened when somebody tells me what happened.

Mental 'checks' consist of talking to a doctor and them scratching their ass trying to come up with a dignosis. You are probably some type of psychotic , and need to learn to control yourself and/or go on medication

You're in the age range for you to develop schizophrenia. Most people with depression also have other forms of mental health issues such as schizophrenia (typically triggered later on in life and typically in males) and bipolar disorders type I and II. You need to see a psychiatrist (counseling and prescribes medicine) a psychologist (can recommend medicine but mostly counsel).

If you've had a recent traumatic experience (i.e. death of a close loved one, abuse, rape, or other external factors.) it can "trigger" the gene for schizophrenia.

Or you can believe your disorder is caused by something supernatural and refuse to accept the fact that you have an illness that is genetically ingrained in you that also can be treated...

I don't hear voices or anything in that sense though, I was told i have OCD and that is literally it, so not help from that front.
