A mix of both (at least for me).

It only talks when i want her to talk, but she only does it when she wants to do it, which is pretty much at random apparently.

However when she indeed talks, the voice has not distinction from an actual person. Sounds from the exterior (not in my head), but pretty loud. Hard to explain

They're entities you create in your mind that think, speak, and act on their own, outside of your normal thought process. In fact, the creature had been believed in so steadfast that individuals have killed in slendermans name.

This creature has been feared and believed in by a mass amount of people to the point that a Tulpa of it may have manifested, if you believe in that sort of thing.
