It's a meme. Well, it is possible theoretically, but that doesn't mean it's realistic in any way. Let me explain. The images you see are rendered by your browser. Some component deep inside Safari is responsible for taking the raw data that is received from 4chans servers and building a human readable presentation of it, so you can look at your porn. If this component had a flaw, that allows certain raw data to be interpreted by it as executable code, not as data--that would mean you could execute code on a computer viewing an image file that was crafted for this purpose.

A real world example for this would be Skype on OSX. Some time ago there was a flaw that would crash Skype when you sent a special string to it (you could only trick it into crashing however, not into executing code). It was fixed rather quickly.

There are almost no bugs of this sort seen in browsers, and what you are talking about is such a freak exploit vector that it can be safely said that the screamer image is just an urban internet legend. In addition, anyone finding such a bug wouldn't waste it on something stupid as that. Those things fetch really big money. But only before they are discovered widely of course!

On that note, let's keep talking about computer-related /x/ things. Regarding OP; Did you know that this kind of rendering-vulnerability is the reason people send spam mails with Word- or PDF-documents? Those renderers are far more complex, and thus there are quite a bit of exploits for them.
