Her name is Akechi Kokoro, and she's from the anime, game, novel, and radio drama "Tantei Opera Milky Holmes." From what I gather, it's a detective story where people are given superpowers by Toys. The fan-made wiki for the franchise didn't have spoilers on anything that happens to her, but TV Tropes doesn't have a horrific death trope, or really anything dark at all, on their page for the series

As far as I can tell, she's still alive and still annoying the rest of the cast

Not sure how she got associated with bandsaw accidents, it must be Google obsessing over somewhere that your reaction image was posted
