The reason why genuine berserker characters are relatively rare in manga and anime is simply in Japanese culture itself. Even in the case of ultra-violence (like Ichi the Killer), they want to depict the act as artful and sophisticated, and at least with a psychological narrative. In the case Guts, he is just one guy instead a bunch of them. The artist and author are trying their best to give him deeper reasons in every possible turn why he is like he is, instead of being just a mindless berserk killer.
You need to resort to Western magic now, anon. There you'll find the Punisher, the Vikings and whatnot. We here in the West are good with these guys as they are. Our heroes can even be guys who fight openly under influence of alcohol and combat drugs. Japanese writers and illustrators seem to not be that much into those things. Certainly there are crazy characters, but it looks like they always want to explain them, often maybe a bit too far.
