Incentive based eugenics is extremely possible and viable. However I think in the case of the picture above, it could be considered recklessly irresponsible or even criminally negligent to allow a genetic disaster to breed as much as she desires, creating all kind of deformed fuckups. They should be sterilized young to avoid any future problems.

That line of thinking is the argument those against eugenics use, against the past retards who wanted to use eugenics against retards. They didn't understand that Downs was largely epigenetic, and most of those with Downs can't breed anyways. Even those few who can have shit fertility rates, and should they manage to have kids, give birth to kids without their condition more often than not. So eugenics doesn't help.
Besides, CRISPR's on the table now - eugenics is old hat.
