Couldn't find the first outside of ancient booru pages with no source.

This one I found under "Neon Genesis Evangelion - Chronicle 26" on the panda. Not sure if that's the correct name.
There is also the illustration in "The Essential Evangelion Chronicle: Side B" but it only has 2 of the Rei's.

If you need it from nippon or other places are out check the buyfag thread/guide on /a/.
Go and get Evangelion Chronicle: Illustrations too if you can.(Rei butt on the cover is well worth the price imo)
