Oh look, it's another anime recommendation thread.
Here's the twist: I want anime recommendations that can meet ALL (not most, not some; ALL) of the following parameters:
>fully subbed in English
>fansubs or CR subs only; no official English subs of any other kind (such as DVD/BD)
>best possible source video (so basically R2DVD rips for older anime, and BD rips for more recent/remastered anime)
>MKV format (don't care much about its intricacies, just so long as it's MKV)
Why am I asking for this? Because when I ask for general recommendations, I always get stuck with some shitfuck format, or encodes from Exiled-Destiny (they almost always use R1 DVDs).
Listing the names of fansub/encode groups is a start, but also a cop-out. If you can recommend specific anime titles from specific groups, you're a better person already.

Come to think of it, most of my problems come from pre-2006 anime. Pretty much all of it is subbed/encoded by Exiled-Destiny. I'm not knocking on their handiwork, but they don't exactly use the best source materials.
Any popular pre-2006 anime you can think of, I am most probably interested in (I missed out on a lot of 90s and early 2000s anime, and I really want to catch up).

Nigga anything made in the last 10 years should fit your conditions. I literally only encounter problems like the ones you mentioned when I look for shows that are 20+ years old.
Stick to anime released after 2005 and you should have no problems.

Also for stuff from the 80s and 90s, unless it's SUPER popular you might have a hard time finding good torrents for it even on BakaBT, unless you have an AnimeBytes account you'll want to do some hunting on IRC. I recommend #intel and #nibl which is where I can find some HD releases for older shows.
