Only trouble is I'm not sure if it is her. She matches Sakuya pretty well but she is wearing a collar around her neck which I don't see in any images of Sakuya.

Just got confirmation from /jp/ that is indeed Sakuya Izayoi, now I'll try hunting down which piece of work it was of her.
