Get MSI Afterburner. Lower your clocks by 20% and see if it helps. If it does help, your card has heavily degraded and needs more voltage to hold normal speeds.
Either give her additional voltage, which makes the card hotter and degrade faster, or lower clocks permanently and sacrifice speed.

Well it's probably software since it showed up in a screenshot. Try >>170034 first. You could try booting into a GNU/Linux live CD/USB drive and see of the problem persists. If it doesn't then reinstall Windows. If it does then your video card is probably fucked.
