After Mythbusters. I doubt it's show-related since they're pretty careful about ear protection most of the time, and nobody else has gone deaf as far as I know. In a recent episode he describes his ears as "basically broken" and because of that couldn't go deep underwater without an air-tight helmet. Presumably the water pressure would do something bad to his eardrums or something.
I assume it's either a consequence of getting old, or possibly a complication of some kind of ear infection or something.

/k/ here. The way an RPG works is it has a conical piece of copper inside of the warhead. While there are anti-personel variants, the standard warhead is a shaped charge. In tanks this is called HEAT. Anyway, when the explosive goes off behind the cone of copper, it heats and pressurizes the copper until it becomes molten, focused in the center of the cone. The molten cone turns into a jet of molten copper that cuts through the armor. The reason an RPG is still deadly in all reactions is because it sill has an explosive charge that explodes in all directions.
