and random spots to produce what you've posted.

Smoothing the hair was easy. You'll have to decide if the nails are OK.
Not much I can do to make the lines finer.
I think that'd require vectoring.

That's a rather vague question and it probably doesn't have a one-size-fits-all answer.
Some people draw elaborate background.
Some go with a gradient fill which draws attention to the central subject.
Others search the Internet for something suitable.

I see you started with a rendered PNG and dropped it onto dead black.
Make sure you can reverse the process and can smoothly render an image which already has a background.
One you have a render, just experiment until you find something _you_ like.
Your taste is all that matters. There's no formula for creativity. If there was, someone would have written an app for it by now. ^_^

You can find tons of images online and tutorials as well. The beauty in this (at least for PS because I use that), is that there is a myriad of ways to do things like patterns, effects, etc. You just have to practice and figure out what works for you.

Yes, start with a square image (try starting small like 150x150) draw your pattern, flatten image, select all, and define pattern or define brush.

I'm sure you can google this or simply look on Deviant Art for brushes/pattern files and tutorials as anon said

Google "Tiled desktop backgrounds" and you'll get tens of thousands.
Or you can make your own. I don't use Photoshop so I can't offer specific advice
but my program has a "tile" function and I'm sure PS has something analogous.
Here, I used the image-sprayer tool to lay down some random cookies.
Then I used "tile" to divide it into a 6x6 pattern. (But the numbers could have been any integers.)
You can make copies semi-transparent, rotate them, interlock them, etc.
There are procedures for making seamless tiles too.

I mostly experiment, but I started out with http://design.tutsplus.com/ for when I was starting out for some neat tricks (plus I took a /gd/ class a couple years ago, consider that if you can). I use IMT for practice too.

if youre using the lasso+wand, always decontaminate colors and create a new later with the "refine edge" option. Otherwise, use the pen tool and create paths. IF you've made a super clean cut, blending options >inner glow > blend as "multiply" in a darker color.

it helps to know what kind of background youre blending onto, since of course this also makes the edges seem blurry. pen tool is the best way - probably

lol, theres actually quite a few spots not erased from the original render. This is what happens when you stack layers on top of each other. It's the nail that made it look weird after lowering the resolution. Hopefully this looks better.
