What happened to people who were sprouted?
Maybe oblivion? I think it's just supposed to be like getting shot with cool flavor for the setting. I really don't think writers obsess over this shit.
Maybe a living excruciating agony, being literally rooted in one spot and having your bones and body ground into nothing by the flowers. Finally release as the flowers die and blow away after completely digesting your bones.
Manny seemed more afraid of it than a normal person would be of being shot, and that it was more socially unacceptable to even own such a weapon than it is to be a known murderer.
Apparently Tim Schafer revealed in the commentary for Grim Fandango Remastered version that those who are sprouted are reincarnated back into the land of the living.

Simpler than that. Bones are a main ingredient in fertilizer, hence why it's flowers.
