Apparently, not one but two teenagers died of heart attacks right after playing this game in separate incidents.
Jeff Daily croaked it in January '81 after posting a score of 16660.
Peter Burkowski met Evil Otto IRL right after posting two top scores back-to-back in October of '82.
Both were apparently healthy young men under 20.
