Probably Robot Attack on the TRS-80. The title screen literally screams ROBOT ATTACK!!!! at you, and it startled me. It was also certainly my first time encountering voice synthesis on the computer, so it was doubly surprising.

2D games were honestly never immersive enough to really scare me, but with the 5th gen I got to play some truly frightening games. SotE was the first to really get to me. The Wampas, the tense fight with IG-88, exploring the sewers, the game is full of terrifying moments. The game also controls kind of clumsily so it has a sort of survival horror feel, especially on the harder difficulties.

Any time I walked into the spider house I'd immediately leave. Didn't know you could get stuff for gold skulltula tokens there for like a year.

Forest Temple also scared me, enough that I stopped playing the game at that point. If I wanted to play more I'd just start over and do the child stuff again.

I can't recall the name of the game, but I remember playing it when I was about three. It was on the NES, and took place inside of a mansion or large house of some kind. When you entered a room in the house, it changed to a first person view, and you had to find stuff in the rooms.

Thing is, when you checked certain doors, monsters would sometimes pop out, and would chase you after you were forced out of the room. One of them was a blue skeletal gorilla or something, and that was the one that spooked me. I don't remember much else about the game, other than it being really hard.

Shadowgate was another one that I thought was kind of scary when I was a kid. I was five when I first played that one. There were so many ways to die in that game, and I probably found most of them. The music and atmosphere were unsettling, and you had to keep an eye on your torches, as well. Once they go out, you're screwed.

At least I didn't play Uninvited when I was five.

The Ooze - on Genesis

It's about this guy who works at a nuclear waste site, and gets restrained and experimented on. He turns into this creepy ooze creature. You play as ooze and have to navigate all these creepy environments and there's mutants and stuff. The music has a kind of creepy vibe to it.

It scared me as a kiddo, so I never played it but a few times.

Low-polygon 3D has such a strange and creepy feel to it that I'm surprised more indie horror games don't use it. Shit like King's Field and even stuff like the Voltigore tunnels from Opposing Force are downright terrifying.

Friend of mine had a 3DO and this game.
I didn't know about "Lovecraftian" horror at the time. From the first moments of this game, I was creeped the fuck out. It only got scarier as we managed to struggle through it. It scared me to the core and totally shaped what I think of now as scary for any media.

Resident Evil seemed like a weak knock off of this to me back in the day. nothing got to me like the original Alone in The Dark again until Eternal Darkness years later.

Some zombie game I walked in on my older cousins playing before I ever even started playing video games.
Didn't scare me enough to run out of the room or have nightmares or anything, but I was like 4-5 and a bunch of bloody 3D zombies freaked me out.

I've always wondered what it was. I think it was a lightgun game and it was the mid-90's.
First game I PLAYED that scared me was Mega Man X. I have a crippling fear of bees that was even worse as a child, so when the giant bee gunship flew down in the first stage I had to turn off my SNES and compose myself for a while before I went back in and toughed it out.

Got myself a copy of Thief when I was 8 and played it without knowledge of what is this game about. After completing the first mission, I thought it's gonna be all about sneaking around people. Then, second mission, I keep checking dead bodies in the mines and suddenly one of them I was bending over starts wheezing and stands up to attack. NOPE'd the fuck out. To this day I think that Thief has the most scary looking and sounding undead enemies.

I think it's the fact that there's so little to do in the area until endgame, the desolate atmosphere and the wild dogs everywhere. The entire place gives off this dreadful feeling, like there's something terribly wrong with it. It's at odds with how happy the rest of town is, too.

Thief: The Dark Project is a great example of that too. The zombies and haunts in that game with the levels that are dripping with atmosphere are way more pants shittingly scary than nuhorror vidya where it's just jump scares in a "hide in the closet" simulator.
