Has there ever been a successful addon system? That's not to say that some aren't great, but generally speaking it seems that they're always doomed from the start.

If you count Japan, the CD attachment for PC Engine did fairly well I think. Can't recall any other success stories though.

Very much this. I think* the first Tengai Makyo game for the CD was a huge hit in Japan, selling hundreds of thousands of copies. Snatcher on the PC Engine sold 100K as well.
Now, these aren't big deals like today's "this game needs 10 million sales to break even", but that's a pretty good return on investment. And given the size of the VG market in the early 90s, if we look at the number of units sold + whatever sales figures exist for the PCE CD, it points to a fairly healthy system up until about 1993-1994.
Pic related is a somewhat unreliable (or impossible to verify) list of the best-selling PC Engine games in 1994. The Tengai Makyo sequel, Manji Maru, is at the #1 spot.
*don't quote me on it, but the number apparently floats between the high 100s to 300,000 copies
