Just made this today, thought I'd share it with you guys. You're gonna need to change the picture settings to the following for it to fit into it on a 1920 x 1080 resolution.
Custom aspect ratio X pos. 436
Custom aspect ratio Y pos. 90
Custom aspect ratio width. 1044
Custom aspect ration height. 772
Integer scale OFF
The .cfg file contains this:
overlays = 1
overlay0_overlay = PVM_Overlay.png
overlay0_full_screen = true
overlay0_descs = 0
Tested with Sega Genesis games picture fits the screen with little to no overscan, no more than an actual TV or monitor would have.

Cleaned up the borders better because I just learned a new cleaner technique, also added a little dark shadow as featured in some tv's for added realism. Please use this improved version.

New one fresh out the oven.
Integer scale ON, 3X scale, core provided aspect ratio. Tested on SNES games and it's an absolutely perfect fit (gotta admit it was totally random, I didn't measure shit.
Make sure overlay opacity is set to 1.00
