My guess is that a lot of the manufacturers had little understanding of videogames or even what exactly the game they were labeling looked like beyond guessing based on the name. They might also have had limited access to artwork to use, so they had to make due with what was available (see: that DuckTales bootleg that uses some generic duck clipart and plain font instead of anything resembling the cartoon). There's also likely some degree of cultural mismatching going on, where the guy designing the label had no idea about the game series or license in question so instead opted for what he thought would be most popular with the kiddies in his target country. With some they probably just slapped on whatever licensed character seemed to vaguely match the concept and ran with it, embracing the view that known character = sales, like slapping a pic of TMNT on a Frogged cartridge or something because they're close enough to frogs.
But of course that is speculation. Some you have to wonder if a human did indeed make it, orat the very least wonder why the most extreme example of "hello my fellow children" was assigned to designing labels when his knowledge of young people entertainment extended to Sonic, Mario, Robocop, and Disney.
