Why is the level design in Bloodlines so bad? I mean, they had three whole years to improve over Super Castlevania IV's amazing level design, so what gives? Did Konami just not give a fuck when it came to quality control on SEGA platforms?

A different team worked on the MD version. During fourth and fifth gen, Konami had the Castlevania IP spread around among various teams. IIRC, Rondo of Blood on PC Engine and the Akumajo Dracula remake on X68000 were developed at the same time.

According to mobygames, only one of the programmers worked on both games.

Nakazato (director, designer and writer of Hard Corps) only worked on SOTN, Dracula X Chronicles and The Adventure (GB) as far as Castlevania goes.

As for Bloodlines' level design, I don't know... the first level is good, because it copies the first level of Castlevania 1, but the rest of the levels have a lot of inconsistent design.

IV and X68K both have much superior level design to Bloodlines.

The gameplay is bad meme is absurd, Castlevania IV's controls are amazing. Different to Castlevania 1? Sure, but so are pretty much every other Castlevania game except for CV2 and CV3 playing with Trevor.
The music is also amazing, in fact every japanese-made Castlevania game has amazing music.
Bosses are hit or miss, but I'd still say they're better than Bloodlines' bosses. Only Bloodlines bosses I like are Moss (really cool boss fight) and Gargoyle (just okay), the rest are very dull and disappointing.
Dracula is OK too because it has 3 forms, but his 3rd form is very easy. What was weird is that his 2nd form felt like a better Death fight than Death itself, which felt like a lazy rehash on IV's Death with a silly rerun of old bosses before it.

I find the levels in Bloodlines more playable than in IV. Versailles has some annoying enemy corridors but that's about it for bad stuff.

it has a lot of bad shit with auto-scrollers and gimmicks, more offensive than the 2 short model 7 parts on IV.

Also, Versailles is actually the best levels on Bloodlines, if not the best, other than the part with the mickey mania-style moving staris. The enemy corridors are fun especially because it has a bunch of unique enemies, and has the best boss fight in the game.

That's actually Drolta, who helped Elizabeth Bathory revive Dracula with her
necromancy magic. She shows up to fight you while Dracula is occupied with
transforming into his second Satan-inspired form, probably to stall you.

Yeah, the gear guy is too static and drawn out for my taste. Bloodlines is a fantastic game but its bosses are kind of lacking, I sort of feel like the first boss is the most interesting even if he is easy.

I feel like it's a bit of an exaggeration, yeah the multi-directional whip makes some things more trivial, but the same can be said about some elements from Rondo or Bloodlines (being able to jump from stairs, for one).
At any rate, the multi-whip is hardly a reason to hate the game. Can understand if a CV1 purist hate it, but can't understand a Rondo or Bloodlines fan hating on IV, they were all easier and "took it too far" from CV1.

The Gargoyle fight is interesting because it kind of forces you to use the diagonal whipping to beat him quickly. And even if his attack pattern is predetermined you're required to react quickly all the itme. One mistake and you can be taken out of the platform and die.

The clock boss just stands there and lets you whip his weak spot until he telegraphs an attack and gives you enough time to avoid it. He feels more like a mid-boss.
